Dr. Nath IS NOT a Florida Medicaid Provider. Patient with Medicaid please contact our office to find out if we participate with your state. Dr. Nath is not a Medicare provider.

The Florida Medicaid Board has decided that Florida has enough brachial plexus specialists to meet the needs of the state and therefore will not accept Dr. Nath's enrollment.

If you are a Florida Medicaid patient, please contact Yolanda at inshelp@drnathmedical.com as soon as you can.

Minors must be registered by their parent or legal guardian.
We will not accept a registration done by a third party.

There is no cost to have Dr. Nath evaluate you at this West Palm Beach outreach event. However, if you make an appointment to see any other doctor in this office, including Dr. Paley - there will be a cost to see them (him). Please discuss cost / insurance etc, directly with Dr. Paley's office staff.
